How do the HMO Proactive Plans and “Tiered Network” plans work?

Tiered or Narrow Network Plans: Under the Affordable Care Act, insurance carriers have introduced “tiered network”, or narrowed network plans. Keystone Proactive Plans have a ” 3 tiered” network of providers.
