The Landscape Today: Ozempic vs. Wegovy

Before we look at the exciting changes coming this summer, let’s clear up the confusion about what is covered right now (as of May 10, 2026).

Ozempic and Wegovy are actually the same active ingredient (semaglutide), but they are FDA-approved for different uses. This distinction is the "secret sauce" to how coverage works.

1. The Diabetes Route (Ozempic)

If you have Type 2 Diabetes, Ozempic is generally covered by Medicare Part D (the prescription drug portion of Medicare). Since its primary purpose is managing blood sugar, it doesn't fall under the weight-loss ban. If your doctor prescribes it for diabetes, you’re usually in the clear, though copays vary by plan.

2. The Heart Health Route (Wegovy)

In early 2024, the FDA approved Wegovy for a new use: reducing the risk of major cardiovascular events (like heart attacks and strokes) in adults who have both heart disease and obesity.

The Big Shift: July 1, 2026 , The GLP-1 Bridge Program

Here is the news everyone has been waiting for. Starting July 1, 2026, Medicare is launching a temporary "Bridge Program" specifically for weight loss.

For the first time in history, Medicare will provide a pathway for beneficiaries to get Wegovy and Zepbound specifically for obesity, even without a diabetes or heart disease diagnosis.

What You Need to Know About the Bridge:

  • The Cost: Qualified beneficiaries will pay a flat $50/month copay.
  • The Duration: This is a temporary program designed to bridge the gap until the permanent system starts in 2027. It runs from July 1, 2026, to December 31, 2026.
  • The Catch: You must be enrolled in a Medicare Part D plan, and your doctor must submit a Prior Authorization (PA) to prove you meet the clinical criteria.

Navigating the Prior Authorization Minefield

Just because the Bridge Program starts in July doesn’t mean you can just walk into a pharmacy and grab a box. You’re going to need to jump through some hoops (the "headache" part of the odyssey).

Medicare and insurance companies will require Prior Authorization. This is essentially your doctor "asking permission" from the insurance company to prescribe the drug. To get approved for the Bridge Program, you generally need to meet these criteria:

  2. Clinical Documentation: Your doctor must show that you are also participating in a comprehensive weight management program (like a reduced-calorie diet and increased physical activity).

The 2027 Transition: Don't Fall Off the Cliff

The Bridge Program is great, but it has an expiration date: December 31, 2026.

If you start your Ozempic or Wegovy journey this summer, you must be prepared for the BALANCE Program which launches on January 1, 2027. This is the permanent, 5-year program that will keep these drugs affordable ($50 copays) through 2031.

CRITICAL WARNING: Coverage will NOT automatically carry over from the Bridge to the BALANCE program.

  • You must select a "BALANCE-participating" plan during the Annual Enrollment Period (AEP).
  • AEP runs from October 15 to December 7, 2026.
