Your EOB helps you understand your out-of-pocket costs when you receive covered services. The new, easier-to-read format lets you quickly find out
how much a provider charged for services, what your health plan paid, and how much you owe. While the attached sample below  is from a specific health insurance company they all follow the same format.  Reading your EOB is especially important if you are submitting your claims for reimbursement through an HRA or health reimbursement arrangement.
